Our Nurse Specialists provide approximately a third of the care delivered by our Hospice across our 700sq mile area. Split into two teams and based at our two centres, these hidden heroes work discreetly in our community supporting patients, families and carers, at or just after diagnosis or at latter stages of a life-limiting illness.
Our Wiltshire Team is led by Nurse Specialist (NS) Simon Devine, and is based in Trowbridge. The 14 NSs work across the patch and in partnership with 28 GP surgeries. Peasedown St John is the base for our 11 NSs that cover B&NES and the Mendip area. They are led by NS Vicky Lear and work with 30 GP surgeries. Both teams work closely with a wide range of health and social care professionals on a daily basis to provide seamless care for the individual. With an average caseload of 14-20, the impact these clinicians have cannot be underestimated. They are often the first point of contact for anyone starting their journey with us.
It is a misconception that Macmillan Nurses are available in our community. Dorothy House is the provider of palliative and end of life care in this community, and we work closely with the Specialist Palliative Care Unit at the Royal United Hospital.
Currently, 21.4% of our referrals are for patients with a non-cancer diagnosis, but our aim is to see this increased to 56%.
