Trowbridge Outreach Centre

Patients can be referred for individual care where they meet the Dorothy House criteria for palliative care, and require specialist palliative care skills to meet their needs. Criteria for groupwork varies for each programme.

This is a group programme aiming to provide information and tools to enable patients to live well and continue doing things that are important to them. Patients are invited to bring a carer, relative or friend with them to the sessions. After an initial assessment, you can attend a course at one of our Outreach Centres and learn about topics including:

• Fatigue management and energy conservation.
• Breathlessness management.
• Sleep.
• Food and diet.
• Physical exercise.
• Relaxation.

The course is informal and relaxed, and gives you a chance to get to know other people going through similar experiences. Refreshments are provided. Each session runs from 10am until 1pm. Following the course, you will be seen again for a review.
