Our Nurse Clinics offer patients and their families an opportunity to visit Dorothy House Outreach Centres in order to gain information on how to maximise their health and well being while coping with a life-limiting illness.
They are an opportunity to hear about Dorothy House services, in an informal environment, and how they may be available to support them now and in the future. They provide the chance to to meet Dorothy House professionals who may be able to answer questions around life-limiting illness and sign-posting to other services.
Who is is for?
• Patients and families who may be reluctant to come to a ‘hospice’ or feel they are not ready for hospice care.
• Patients which need lighter touch contact e.g. due to having a long term condition/chronic illness.
• Patients who do not have complex or acute issues needing clinical input.
• Patients who are able to independently travel to the outreach centres at Trowbridge and Peasedown.
To make a referral to the Dorothy House Nurse Clinics, you will need to ask a health professional i.e. GP, to complete our referral form and send to the Dorothy House Clinical Coordination Centre. If you would like to discuss a referral please call the Clinical Coordination Centre on 0345 0130 555 (Monday-Friday, 8-6pm; Saturday-Sunday 9am-5pm).
