Hospice at Home
Our Hospice at Home service provides palliative care in the later stages of someone’s life. Our experienced healthcare assistants support patients and their families in their own home, regardless of whether the patient lives alone or with others.

Respite care
The service is designed to offer respite to allow carers to get a good night’s sleep. It also allows quicker hospital discharge and helps prevent unnecessary hospital admissions.
We also offer Hospice at Home care in the last few days of a patient’s life. This allows patients to remain at home, if they wish.
The team provides personal care and will sit in the same room as the patient, or a room close by, if preferred. Carers use room monitors and carry out regular, discrete visual checks to monitor a patient’s condition.
Referrals
We offer Hospice at Home in conjunction with other services. Patients will be referred by one of our Nurse Specialists or another healthcare professional. For example, a member of the Specialist Palliative Care Team and Integrated Discharge Service at the RUH.

Monday-Friday: 8am-6pm
Saturday-Sunday: 9am-5pm
