Charitable Trusts help us take our care to more people

Generous grants totalling £85,000 from the Garfield Weston Foundation, the Albert Hunt Trust and the Underwood Trust are helping us evolve and enhance our care to help us reach more people across our community.

In 2022, we launched 10 new ‘Community Palliative Care Teams’ to expand our community nursing model and help us care for significantly more patients each year.

Each of our 10 new teams is dedicated to caring for patients at home in a specific locality, and consists of health professionals with a mix of complementary skills. They also coordinate patients’ care with our wider hospice services and liaise with local volunteers to ensure patients and families receive practical and social support.

The delivery of the first year of this exciting development has been supported by several Charitable Trusts and Foundations, who have recognised the importance of our care evolving in this way.
