Many of us need something positive to focus on when we’re dealing with grief and loss after the death of a loved one. A Tulip Fund is a meaningful way to remember someone special and support the Hospice. Each Fund allows us to continue to provide care for our patients and their families.
Setting up a Fund is easy and anyone can do it. Once the Fund is live friends, relatives and colleagues can make single or regular donations, and even add sponsorship from fundraising activities. The Fund can grow from year to year as a lasting tribute for someone you loved.

- Download and print the form here
- Once you’ve completed the form, post it back to us
- When we receive your form, we’ll send you a confirmation letter
- Then every year, on the anniversary of the start date of your Fund, we’ll post a certificate to you so you can see you how your Fund is growing
