Our care

We provide care as close as possible to where our patients live. Our ten Community Palliative Care Teams cover a 700 sq. mile area, providing physical, psychological and spiritual support. Experts in providing specialist palliative care, our staff include doctors, nurses, therapists, carers and counsellors and will see patients at home, in a clinic setting or in a care home. They work closely with GPs, community nursing teams, hospital staff and other healthcare professionals to ensure a high standard of care and support. We are regularly inspected by the Care Quality Commission.

 

Our services, described below, require a referral which can be made easily by any health and social care professional, by a person with a life-limiting illness (self-referral) or by a friend/family member of a person with a life-limiting illness. Please see Referrals for more information.

We also provide Groups and Workshops open to anyone in our community who may be affected by a life-limiting illness or bereavement. No referral is required to attend these regularly held sessions.
