Advance care planning

Good palliative care is deeply meaningful work as it is our last chance to do right by our patients. We all suffer when we get it wrong.

As a health and social care professional you play a crucial role in supporting patients to consider their end of life wishes. By facilitating and recording these important conversations you can help to reduce the number of end of life crisis – distressing and resource intensive emergencies, and improve staff morale and patient care.
