Our care
We provide care as close as possible to where our patients live, covering BaNES (Bath and North East Somerset), and parts of Somerset and Wiltshire. Our ten Community Palliative Care Teams covers this 700 sq. mile area, providing physical, psychological and spiritual support.

Specialist palliative care
As experts in providing specialist palliative care, our staff include doctors, nurses, therapists, carers and counsellors and will see patients at home, in a clinic setting or in a care home. They work closely with GPs, community nursing teams, hospital staff and other healthcare professionals to ensure a high standard of care and support. We are regularly inspected by the Care Quality Commission.
Referrals
Our services, described below, require a referral which can be made easily by any health and socialcare professional, by a person with a life-limiting illness (self-referral) or by a friend/family member of a person with a life-limiting illness. Please see Referrals for more information.
We also provide Community Groups open to anyone in our community who may be affected by a life-limiting illness or bereavement. No referral is required to attend these regularly held sessions.
